The original organ for Fifth Avenue Prebyterian Church was built in 1855 by Geo. Jardine & Son of New York City. When the present church was built in 1873-75, Jardine was asked to move and rebuild the organ, adding the 32' Double Diapason which is still in use today. In 1893, a new organ of 3 manuals and 44 stops was built by John Odell of New York City. Both the Jardine and Odell organs were powered by a hydraulic motor—a steam engine pumped water to a tank above the tower's E. Howard & Co. clock, then the water flowed by gravity to a basement cistern, operating the feeders of the bellows. A gradual conversion to electric power followed. In 1913, Ernest M. Skinner of Boston was awarded the contract to build his Opus 206, a new four-manual and pedal organ for the Sanctuary plus a two-manual organ for the Lecture Room. Skinner also carved the magnificent non-speaking case which strongly resembles that of the organ in St. Bartholomew's, Armley, Leeds. Skinner altered the organ in 1945 and again in 1955. In 1961, Austin Organs, Inc. of Hartford, CT installed the present organ.

Noteworthy organists at Fifth Avenue Presbyterian Church have included William Mason (Lowell's son), Eugene Thayer, Harold Gleason, W. Lynnwood Farnam, Robert S. Baker, Richard Bouchett, William Whitehead, and Terence Flanagan.

The present Sanctuary Organ was built by Austin Organs, Inc. of Hartford, CT, in 1961 as their Opus 2347. It originally had 120 ranks, 18 of which were retained from the previous E.M. Skinner organ. The 1959 contract, totalling $132,300, called for a newly-designed Austin drawknob console with a capture combination action and imported English ivory keyboards, and modifications to the Skinner case to provide room for a choir and instrumentalists. Dr. Robert Baker, organist and choirmaster of the church from 1953-62, served as consultant. The Fifth Avenue organ was one of a "quartet" of large and important Austin organs installed in the 1960s in New York City, the others being at Brick Presbyterian, First Presbyterian and the Church of the Heavenly Rest.

In 1996, a new solid-state 4-manual drawknob console was built by the Wicks Organ Company of Highland, IL, retaining the ivory keyboards from the previous Austin console. In 1998, the Austin Organ Company rebuilt and releathered the mechanisms of the instrument. Daniel L. Angerstein tonally revoiced the organ in 1999, replacing several pipe ranks and providing new digital ranks.

In 1913, a new organ was built by Ernest Skinner, who also carved the present organ case. The choir loft built into the case was at this time much smaller, housing only a quartet of singers used to lead the congregation.
